What is supported living?
Supporting living enables individuals to live as independently as possible within their own homes and
communities by having a support network in place to work with them. The amount of support in
place is dependent upon particular needs, ranging from only a few hours a week, to 24 hour support
and/or care.
What we provide
• Support services for individuals within their own tenancies following discharge from long stay
hospitals, progression from family homes, or transition from children's services.
• Varying levels of support determined through our assessment processes with input from the
individual and their current support network. Continuous partnership with multidisciplinary
teams is undertaken to review support / care plans ensuring that we best meet needs and
support development.
• Carefully matched staff. This is done so that they fully understand the individuals bespoke
support needs and to ensure that they can build up effective long term rapports.
• Accommodation as well as the support service if needed. In all cases, we ensure that wherever
the individual is placed, it is where they want to live and is conducive to their needs.
• Community inclusion and opportunities for education, social and leisure activities, training and
development.
• Home visits to family and friends.
- Domiciliary Care
What is domiciliary care?
Domiciliary care assists individuals with their personal care and supports them in carrying out practical
household chores within their own home. Such care helps individuals to remain as
independent as possible.
The type and level of domiciliary care is dependent upon the needs of the individual. This may
vary in terms of constant assistance, short term assistance or only occasional assistance. Care
may be needed as a result of being in hospital or through having an illness or disability, resulting
in the need for occasional help.
What we provide
• Care services provided within an individual's own home.
• Personalised care packages that best promote the independence of the individual.
• Varying levels of support determined through our assessment processes, with input from the
individual and their current support network. Continuous partnership with multidisciplinary
teams is sought to review care plans ensuring that we can best meet needs and help them live as
independently as possible.
• Carefully selected care staff who are matched to the personal needs of the individual and who
are able to build strong and trusting rapports.
• Personal care including; dressing and undressing, bathing, washing, shaving and oral hygiene,
toilet and continence requirements, medication requirements, manual handling, assistance with
eating and meals.
- Transition Support
What is transition support?
Transition support addresses the needs of adolescents and young adults with physical and / or
medical conditions as they move from child-centred to adult focussed health care systems.
Transition support addresses the needs of the young person in relation to their psychosocial,
medical and vocational requirements to help them with their personal development.
What we provide
• A service that aims for the young person to achieve their full potential in terms of health,
education and personal development by promoting control of their own social and emotional
development.
• Support that makes the transition from childhood to adulthood as smooth as possible for the
individual.
• Support that plans and considers aspirations for the future as well as short term needs.
• A support network with a number of agencies to present conducive opportunities for
development.
- Outreach
What is outreach?
Outreach support is available for individuals who need assistance on a more occasional basis. It helps
the individual to perform daily activities and access local activities which they would not be able to without
support. The level of support will vary depending on the needs of the individual, but can range from
a few hours a week, or a few hours daily.
What we provide
• Support on an ad hoc basis to allow individuals to access occasional activities and amenities such
as education, employment, training, healthcare, social networks and shopping. This allows the individual
to remain in their own homes whilst accessing community activities.
• Support to build and maintain community connections and personal relationships.
• Support with activities within the home such as housekeeping, cooking, shopping and
household budgeting.
- Therapeutic Services / Placements
What are Therapeutic Services?
Therapeutic Services provide a systemic range of clinical services to meet the differing and
complex needs of the young people we support.
The service is lead by our partner Senor Clinical Psychologist, who has over seven years
experience of delivering psychological services to looked after children and youth offenders.
What we provide
• A comprehensive psychological assessment incorporating nationally recommended
validated outcome measures.
• Expertise in; Cognitive Behavioural Approaches, Trauma Therapy Systemic Approaches,
Assessment and treatment of Harmful Sexual Behaviour, Systemic Approaches, Violence Risk
Assessment, Substance Misuse Interventions and Approaches for Emotional Regulation.
• Specific training packages for our support staff developed through the psychological assessment,
ensuring that the young person receives support of the highest possible quality to contribute to their development.
• Experience in working under Multi-Agency Public Protection Arrangements (MAPPA) and
Child Protection Strategies.
