Our Aims

Rialto Care's aim is to plan and deliver a service that is highly individualised and Individually centred to each person and will be able to meet the complex and differing needs of each individual. The services provided to each client will be planned on the accurate information gathered from a range of assessments delivered from a multi-disciplinary agency consisting of clinical and social care teams.

We will visit, and utilise where applicable the various models of care to enable us to identify what a person's individual and preferred lifestyle is and how we can help them achieve this.

• We aim to maximize all Service Users quality of life
• We aim to ensure that all Service Users have their rights met
• We aim to ensure that all Service Users have their physical well-being met by well- Wo/men groups, meeting with local healthcare practitioners
• We aim to keep each Service User safe from; harm, exploitation, injustice and all forms of discrimination
• We aim to ensure that all people receiving support from us will have access to all opportunities and amenities which they require
• We aim to provide all people receiving support from us with an identity where their ideas and views are valued and acted upon
• We aim to provide Service Users with free access to all of their information, continually protecting his/her interests and confidentiality
• We aim to maximize all Service Users benefits
• We aim to, where possible, assist in maximizing and promoting the Service Users autonomy
• We aim to document all progress made, and strive to promote further self-autonomy

A key to ‘involvement' is the commitment by all team members to enable Service Users to have the ability to communicate, express their individual choice and to make autonomous decisions.
